Our office is usually open from 8:30 a.m. to 6:00 p.m. We answer the phone only between 9 and 5. Your call is very important to us.

When you call, let the receptionist know whether you are scheduling with Peter or Lisa. You may download your Free "Welcome Packet" which contains important information for you to fill out before your first appointment. Simply click on the appropriate document links below and print them out.

For Adults and Teenagers (13 years and older) download these 4 forms:

  1. Welcome Letter - Read only

  2. New Patient Intake Form - Fill out and bring with you

  3. Adult Consent Form - Read, sign and bring with you

  4. Financial Policies Form - Read, sign and bring with you



For Pediatric Visits (12 years and younger*) download these 4 forms:

  1. Welcome Letter for Parents - Read only

  2. Pediatric Intake Form - Fill out and bring with you

  3. Pediatric Consent Form - Read, sign and bring with you
